The shrine is dedicated to the Virgin of Grace and was built after the apparition (1480) of Mary to two shepherds of Baragazza, a boy and a girl. The girl became a nun (Sister Cornelia) and donated an Andrea della Robbia relief depicting Mary with the Child to the church. In the 17th century, the church was enlarged as a project of Angelo Venturoli, an architect from Bologna. Its façade is edged with a porch.
